Care Coordination Services
Health Homes is a free, voluntary care coordination model that promotes and facilitates communication between all caregivers involved in a patient’s care, to address all of a patient’s needs in a comprehensive manner. Members of Essen’s Health Homes Program work with personal Care Coordinators to obtain the tools they need to take back their health.
Employer Health Services
Essen Urgent Care knows the importance of healthy employees for healthy work environment. After all, your employees are your greatest asset. New York Research Education and community health is a non for profit wing of Essen Health Care which is involved in academic training and community health in the underserved communities
Nursing Homes
Nursing home division consists of over 100 providers who provide medical care to nursing home residents across new York City boroughs, Long Island and Westchester. Our vision is to create an integrated system of care to deliver high quality accessible and cost effective medical services for nursing home residents.
Hospitalist
Essen Health Care provides hospitalists and physician services in nursing homes to provide a complete spectrum of care delivery in a clinical setting.
Atlas IPA
Atlas IPA is part of practice management of Essen Health Care. It leads payors negotiations, and support the practice with credentialing and IT solutions needed for the optimal care of patients. It also supports providers within and outside Essen with compliance regulations, administrative contracts and other overhead issues. The IPA aligns providers with a variety of goals including improving quality of care and reducing cost.
